Stenosis is when there is an actual narrowing of the spinal canal,preventing normal spinal movement. or when the canal at a particular level is narrowed by bony or other structures so as to obstruct CSF flow. It can also be defined according to symptoms – stenosis of neurogenic origin (narrowing due to compression), and stenosis of vascular origin (a narrowing caused by constriction in one of the arteries feeding into it). As the spinal cord becomes compressed, it becomes harder for the brain to communicate with the rest of the body. The main symptom of stenosis is often pain in the lower back, buttock and legs. However, stenosis can also cause tingling, numbness, muscle weakness or spasms, or even bowel and bladder problems.
How does stenosis cause pain?
Pain can be caused by direct pressure on the spinal cord, nerve root irritation or compression, inflammation, or even nerve damage. The pain can be intense and may radiate across different parts of the body. The symptoms are often felt in your back, buttocks, hips, or legs. Nerve root irritation typically causes a stabbing pain that is worse with movement and improved by rest; inflammation usually creates an aching sensation that is worsened by activity but relieved when you lie down; nerve damage has shooting pains like lightning bolts which come without warning or pattern to them In stenosis, the nerve root itself is usually fine, but the bony protrusions that round the nerve root are too narrow, which impairs the spinal canal, preventing normal nerve signals. This results in localized nerve damage, pain, and functional problems for the affected area. The nerves that are most commonly affected by stenosis are the sciatic nerve, the spinal nerve roots, or the dorsal root ganglia, which are the nerves that control the muscles and organs in the lower back and pelvic area.
